- Respond to emerging community health needs by supporting innovative, community-centered solutions.
- Elevate and address intersectionality to ensure access and opportunity.
- Support projects that share power with community through intentional inclusion and co-design.
- Partner with organizations focused on changing inequitable systems, practices, and services.
Funding Information
This opportunity can support projects from 6-months to 5-years with a request amount ranging roughly from $5k – $500k+.
Uses of funds
Funds may be used for a variety of health equity focused efforts, including but not limited to:
- Programs/services
- General operating support (up to 18-months)
- Compensation (e.g., salary, stipends)
- Advocacy efforts
Funds must support people and communities that have been historically marginalized and are impacted by health inequities. This may include, but is not limited to:
- Communities of color/Indigenous communities
- Children and/or adults impacted by poverty
- Persons with disabilities
- LGBTQIA+ persons
- Rural communities
- Refugees/New Americans
- Older adults.
Eligibility Criteria
Eligible organizations:
- Current and past grantees are eligible to submit.
- Fiscal sponsors: If you are not a non-profit, you may use a qualified 501(c)(3) organization as a fiscal sponsor.

- Proposals submitted must be completed by the entity seeking funding, not the fiscal agency.
- A non-profit that acts as a fiscal sponsor for another entity is eligible to submit on behalf of their organization.
