Grantmaking Criteria
Projects will be evaluated on the following criteria. Strong programs will:
- Enhance health or prosperity of individuals in at least one of the 24 counties served by Common Ground Healthcare Cooperative
- Articulate a well-defined need with clearly defined actions to meet the need
- Build organizational capacity and lead the program to become self-sufficient
- Use collaboration to leverage impact
- Be feasible and likely to succeed
- Have a reasonable level of risk
- Propose an accurate, appropriate and reasonable budget
- Attract additional funding and leverage resources
- Clearly define meaningful and measurable project outcomes and methods to report actual outcomes
- Have a long-term impact, especially relative to cost.
Eligibility Criteria
All applicants must meet the following:
- Be a nonprofit organization with a 501(c)(3) designation by the Internal Revenue Service or a governmental body, such as a school or municipality, or an organization that is not tax exempt under I.R.C. § 501(c)(3). Grants to entities that are not tax-exempt under I.R.C. § 501(c)(3) shall be made only if the grantee organization agrees in writing to keep the funds in a separate fund dedicated to purposes described in I.R.C. § 170(c)(2)(B)
- Directly or indirectly serve the individuals within the Common Ground Healthcare Cooperative service area, Brown, Calumet, Dodge, Door, Fond du Lac, Green Lake, Jefferson, Kenosha, Kewaunee, Manitowoc, Marinette, Milwaukee, Oconto, Outagamie, Ozaukee, Racine, Shawano, Sheboygan, Walworth, Washington, Waukesha, Waupaca, Waushara, or Winnebago counties
- Conduct business without discrimination on the basis of race, religion, color, sex, gender identity, sexual orientation, age, marital status, disability, veteran status or national origin.
