Details:

The purpose of this grant is to facilitate the equitable distribution of funds among community-based organizations and agencies that support the goals and strategies outlined in the R-CHIP. The key objectives include:

  • Strengthening the Linn, Benton, and Lincoln Counties progress in achieving the R-CHIP goals and strategies.
  • Ensuring a cohesive and complementary approach to addressing public health priorities.
  • Supporting data-driven, evidence-based strategies.
  • Expanding the awareness and implementation of the R-CHIP across agencies and organizations in Linn, Benton, and Lincolin Counties.

Priority Areas

Proposals must align with at least one of the following R-CHIP priority areas:

  • Access to Affordable Housing
  • Access to Quality Care
  • Behavioral Health
  • Inclusion, Diversity, Anti-Racism, and Equity (IDARE).

Funding Information

  • Grant Amount: Up to $200,000 for Linn or Regional (Must be inclusive of Linn County)
    • $25,000 Programmatic Grants (4 awards):
      • Supports specific projects, services, or initiatives that directly address community health priorities and provide measurable outcomes. These funds are intended for activities such as direct service delivery, community education programs, and outreach efforts.
    • $50,000 Capacity-Building Grants (2 awards):
      • Designed to strengthen organizations by enhancing their infrastructure, workforce, partnerships, or long-term sustainability. These funds can support activities such as staff training, technology upgrades, and strategic planning. This funding supports specific projects, services, or initiatives that directly address the community health priorities and provide measurable outcomes.
  • Grant Amount: Up to $60,000 for Lincoln, Benton, or Regional
    • $15,000 Programmatic Grants (4 awards):
      • Supports specific projects, services, or initiatives that directly address community health priorities and provide measurable outcomes. These funds are intended for activities such as direct service delivery, community education programs, and outreach efforts.

Uses of Fund

Funds must be used to support projects and initiatives that align with the R-CHIP goals. Eligible activities include but are not limited to:

  • Program implementation that addresses public health needs.
  • Capacity-building efforts to strengthen organizational infrastructure.
  • Data collection, evaluation, and strategic decision-making processes.
  • Community outreach and engagement efforts to promote health equity.

Eligibility Criteria 

  • Applicants are encouraged to align their proposals with key strategies from the R-CHIP that are best suited for this funding opportunity, including but not limited to: The following entities are eligible to apply:
    • 501(c)(3) Nonprofit organizations
    • Faith-based organizations, including churches
    • Government entities, including municipalities and public health departments
    • Organizations working in the Linn, Benton, and/or Lincoln County Regions.
  • Clarifying Regional Eligibility:
    • Funding is available for both county-specific and regional efforts—with the important requirement that funded work must include meaningful activities in one or more of the priority counties, as outlined below:
      • Linn County or Regional: Organizations may apply for funding to support work that takes place specifically in Linn County or spans the broader region. However, to be eligible, the proposed work must include activities or services within Linn County.
      • Lincoln, Benton, or Regional: Organizations may apply for funding to support work that occurs specifically in Lincoln County, Benton County, or across the broader region. To qualify, the proposed work must include activities or services within either Lincoln or Benton County.
