Through this fund, they support organizations that are making a real difference by expanding access to health care and addressing the everyday factors that shape people’s well‑being, like nutritious food and stable housing.
The program is designed to support organizations serving people who face the highest risk of poor health outcomes, particularly those related to chronic conditions (such as cardiovascular disease and diabetes), mental and behavioral health conditions, and substance use disorders. They prioritize nonprofits that improve outcomes for these individuals through accessible clinical care, social services, or community-based support. This year’s program will consider applications from organizations working to expand access to one or more of the following:
- Community support and education programs for people with existing chronic and/or mental/behavioral health conditions
- Mental and behavioral health care in clinical settings for people with existing mental/behavioral health conditions
- Nutritious food and meals for people experiencing food insecurity
- Safe housing and support for people experiencing homelessness
Funding Information
Each selected organization will receive a $50,000 grant award for a 12-month period.
Eligibility Criteria
Through this initiative, the Foundation intends to fund:
- Registered 501(c)(3) organizations that are both located in and directly serving residents of:
- Connecticut: Towns and cities within a 20-mile radius of the city of Hartford
- Rhode Island: All towns and cities
- Organizations with annual operating budgets between $250,000 and $15 million
- If applying through a fiscal agent, the fiscal agent must hold its own 501(c)(3) status and manage all legal, financial, and reporting responsibilities tied to the grant
