Each year, the committee uses the CHNA to inform how it collaborates with nonprofit organizations in Orange, Osceola, Seminole and South Lake counties that are working to create lasting improvements in community health and economic stability for Central Florida’s residents.
Eligible projects should demonstrate a direct impact on community well-being, healthcare access and support initiatives that improve Social Determinants of Health – specifically Housing Instability, Transportation and Food Insecurity.

Funding Information
The maximum funding range is around $100,000 annually.
Eligibility Criteria
Applicants must demonstrate long-term individual or family stability, and expansion by meeting one of the following criteria:
- Increase in unduplicated individuals served beyond current client base
- Addition of a new target population
- Expansion into new geography or area
- Development of cross-sector coordination
- Creates measurable improvements to current processes or access.
