The purpose of the Perinatal Telehealth program is to develop or expand innovative perinatal telehealth models that strengthen Tennessee’s maternal health system, reduce gaps in access to specialty obstetrical care, and support a more coordinated statewide approach to high-risk perinatal services. Through this RFGP, the State seeks to identify partners capable of addressing these challenges through integrated telehealth models grounded in evidence-based practice, strong partnerships, and sustainable system design.
The State intends to invest in projects that strengthen coordination across Tennessee’s perinatal system by supporting telehealth models that align with statewide priorities and integrate seamlessly with existing resources. The projects should not only expand clinical reach but also build durable infrastructure, enhance communication across providers, and support the perinatal workforce through teleeducation aligned with statewide best practices. Projects funded should serve as building blocks toward a more unified perinatal telehealth system capable of improving patient outcomes, decreasing travel burdens, strengthening referral pathways, and supporting providers in rural and underserved communities.

Criteria
- The State is seeking proposals that strengthen access to maternal-fetal medicine specialists and other clinicians involved in high-risk obstetrical care, with a particular focus on geographic areas experiencing significant maternal and infant mortality and
morbidity. The proposals should include comprehensive, integrated perinatal telehealth projects that bring together clinical telehealth service delivery, provider-to-provider consultations, and tele-education activities that directly reinforce clinical care.
Proposers are expected to address system-level challenges such as maternity care deserts, limited availability of specialty care, and barriers to adopting telehealth technologies. - Each proposal must clearly describe how the model addresses the needs of the geographic area it serves, leverages community and clinical partnerships, enhances care in regions with elevated maternal and infant mortality and morbidity rates, and contributes to a cohesive, statewide vision for perinatal telehealth.
- Perinatal clinical service delivery under this RFGP and the Sample Grant Contract refers to the use of telehealth technologies to provide direct clinical healthcare services to high-risk pregnant and postpartum patients. These services must include telehealthbased clinical obstetric care such as maternal–fetal medicine consultations, remote patient monitoring for obstetric conditions during pregnancy and postpartum, and remote obstetric ultrasound services when appropriate and clinically feasible. Each project should also enhance provider competency through high-quality tele-education, training, and sharing of best practices.
- Proposals must demonstrate a clear understanding of Tennessee’s perinatal system, identify gaps within the proposed service area, and outline a telehealth model that contributes meaningfully to the long-term vision of improved maternal and infant health outcomes across the State. Proposals should also demonstrate how the project will improve care coordination through clearer referral pathways, consistent follow-up processes, and strong connections with Regional Perinatal Centers.
