Case Management Services address the needs of at-risk residents, primarily lower income individuals and families. Case management services provide a single point of accountability for coordination of services and are comprehensive to address the complex needs of the populations served. Case management includes the documentation of services and progress, regular meetings with participants, and connections to additional supports to ensure self-sufficiency and wellness.
Eligibility Criteria
- Comprehensive case management services that provide a single point of accountability and coordinated access to multiple services to address the needs of individuals/households; and target at-risk residents
- Target primarily low/moderate income residents (<80% AMI) with greatest needs and barriers to accessing services, using a racial equity lens
- Documentation of services and progress toward case plan goals, regular meetings with participants, and connections to additional services needed to ensure self sufficiency and wellness for at-risk residents, primarily lower income individuals and families.
