Application Requirement
Please include the following information:
- Organization name, address and contact information
- Project name, purpose and description
- Project budget breakdown and organization’s current balance sheet and income statement
- Expected timing and impact of project on people and community
- Demonstrated local support for project (financial and personnel)
- How project will be sustained in the future
- Project team, past successes/proven ability
- Dollar amount requested and time frame for disbursement
- 501 (c) (3) information.
