This program is expected to result in adoption of evidence-based chronic disease prevention and health promotion approaches and effective use of public health tools and resources. This 5-year NOFO supports strengthening the leadership and expertise among chronic disease health departments to transform the practice of chronic disease prevention and health promotion in state health departments and accelerate improvements in population health. Focusing on the priorities and goals of NCCDPHP, this program will support the development of effective state chronic disease programs through implementation of practice-based learning and leadership development strategies.
This NOFO will focus on strengthening the capacity and capabilities of the public health workforce, with particularly focus on health department staff across the United States, territories, and freely associated states. It will support the development of effective state chronic disease programs through peer-to-peer meetings, practice-based learning and leadership development activities for chronic disease leadership and staff, and outreach and networking with local health departments. This NOFO will result in better-equipped state health department chronic disease leadership, improved ability to coordinate across program areas and with non-traditional partners, and engagement in strategic workforce planning that addressed changing technology, an aging workforce, and evolving public health workforce needs.
Outcomes
During the 5-year period of performance, the recipient is expected to implement the cooperative agreement strategies and activities to achieve the cooperative agreement outcomes. The activities performed under this cooperative agreement should work toward the short-term outcomes outlined in the logic model, including:
- Increase chronic disease public health staff access to resources and tools to address leadership, chronic disease prevention approaches and priority topics,
- Improve chronic disease public health workforce knowledge, skills, and competencies that support public health, including Social Determinants of Health and Health Equity, and
- Improve public health workforce knowledge of workforce structures and systems that are flexible and responsive to changing public health needs
