Request for Proposals for Regional Health Equity Network Grant Program (Minnesota)

The Minnesota Department of Health invites funding proposals from nonprofit community-based organizations, Tribal health departments, public health departments, and community health boards that will connect, strengthen, and amplify health equity efforts.

Details:

The purpose of this RFP is to strengthen regional and community partnerships that advance health equity. It is a component of the Regional Health Equity Network Project. The RFP will fund activities and projects that shift conditions that hold inequities in place and open pathways to advancing health equity. All funded projects must intentionally build relationships that are foundational for successful public health partnerships.

Project Categories

There are three funding categories (applicants may choose more than one category). Funding categories and example activities are listed below. Applicants are encouraged to link their activities to one or more of the conditions of systems change.

  • Funding Category 1 – Connect: Building and initiating new relationships among local public health, Tribal public health, community organizations, and/or others advancing health equity. Example activities include, but are not limited to:
    • Connecting and building relationships with regional equity partners
    • Building inclusion of populations being served in planning efforts & keeping in mind how to sustain such inclusion efforts
    • Incorporation/utilization of diverse media outlets for public health efforts
    • Cross sectoral relationship building within a county, Tribe, and/or region
    • Support the development of a program by building relationships with communities most impacted by health inequities and key partners
    • Support equitable representation on advisory groups, community councils, or similar including individuals with lived experiences and individuals from communities being served
    • Create equitable community engagement opportunities for community members with lived experience to co-create, share decision making power, and implement actions that advance health equity
    • Conduct health equity focus groups to build relationships with specific populations and better understand specific inequities impacting the population
    • Increase understanding of health inequities experienced by communities through targeted assessments, behavioral health surveys, or other data collection method(s)
    • Create shared opportunities to learn how to effectively address health inequities in communities
  • Funding Category 2 – Strengthen: Strengthen organizational and community capacity to advance health equity through cross-sector training, technical assistance, and skills development. Example activities include, but are not limited to:
    • Increase understanding of health inequities experienced in rural communities as a means to strengthen work to advance health equity in rural settings Conduct a Health Equity Organizational Assessment
    • Adopt and implement a Health Equity Policy Review process
    • Promote diverse workforce representative of the communities being served
    • Provide implicit bias training for staff from two or more organizations
    • Implement indigenous evaluation methods
    • Improve quality of connections and relationships among partners working on health equity issues
    • Strengthen relationships and connections that fosters a shift in power dynamics
    • Examine system structures, policies, practices, cultural norms and values in order to create inclusive and equitable communities
  • Funding Category 3 – Amplify: Amplify existing relationships and the interconnected work of communities and public health systems to prioritize community solutions that advance health equity. Example activities include, but are not limited to:
    • Embed a Community Coach or individuals/elders from a community to support equity practices
    • Extend funding for an existing Community Health Worker position
    • Build on COVID-19 partnerships that reached communities most impacted by health inequities to address other public health improvement efforts
    • Broaden the use of cultural practices to support health outcomes
    • Embed health equity practices into engagement efforts
    • Amplify partnerships across sectors by developing shared tools and language around equity as it relates to data
    • Support ongoing data equity dialogue through training and work to integrate into equity practices
    • Collaborative solutions that address systemic inequities
    • Embed learnings into practice with goal of achieving system-wide changes that advance health equity

Funding Information

  • Estimated amount to grant: $5,000,000
  • Estimated award maximum:$150,000
  • Estimated award minimum: $20,000

Project Dates

This second round of grants is anticipated to start July 1, 2023. The end date for all projects is May 31, 2024.

Eligible Applicants

Eligible applicants will work towards advancing health equity in their communities or within their region within the state of Minnesota. Eligible applicants include:

  • Community health boards
  • Nonprofit community-based organizations
  • Public health departments (city or county)
  • Tribal governments

Eligible applicants must be registered with the Minnesota Secretary of State and not suspended or debarred by the State or the Federal Government. Eligible applicants can apply in each round; however, applicants are limited to receiving a total of $150,000 between both rounds. Applicants are not eligible for $150,000 in each round. Nonprofit community-based organizations applicants must partner with either a Tribal health partner or a local public health department on their application and must be prepared to provide written verification that the Tribal health partner or community health board partner is a project partner, as requested, during the grant agreement process.
