Purpose
- People living in low-income ZIP codes in the city of Rochester have a first-dose vaccination rate slightly above 3 percent, while the average for the rest of Monroe County is four times higher, with rates in Brighton and Pittsford topping 22 percent.
- Neighborhood associations and community-based groups play an important role — building relationships and grassroots collaborations and proposing solutions to issues affecting their residents. These groups are the first to know what is happening in their community and have become trusted partners for local businesses and government entities in reaching out to their neighbors.
- Building off their Neighborhoods Grants initiative, which provides financial support to city neighborhood groups to strengthen the communities in which they live, Rochester Area Community Foundation wants to help residents in Rochester’s low-income neighborhoods better understand the importance of continuing to be tested for COVID-19, and how and where to access the vaccination. The goal is to improve vaccination rates in these neighborhoods and share information on how citizens can protect themselves and their family and friends.

Eligibility Criteria
- Neighborhood-led and community-based organizations.
- Must collaborate with a COVID-19 resource organization (such as the Black Physicians Network, Interdenominational Health Ministry Coalition, Rochester Black Nurses Association, Monroe County Health Department, etc.) that is not necessarily a government entity or health-specific business or nonprofit.
- Together, the neighborhood group and COVID-19 experts will determine the best and most innovative ways to reach residents and share important information related to COVID testing and vaccines to improve vaccination rates and decrease COVID cases.
