Agencies that have the ability and capacity to provide services to people living with HIV from racial and ethnic minority groups and to increase enrollment in Special Pharmaceutical Benefits Program of eligible HIV-positive individuals who are at high risk or have been lost to care are encouraged to apply.
Funding is available for the following:
- Minority HIV/AIDS Initiative (MAI)
- Special Pharmaceutical Benefits Program (SPBP)
The funds available will be used to deliver an innovative program in order to accomplish the four complementary purposes detailed below:
- Grant awards to AIDS Service Organizations (ASOs) to engage HIV-positive individuals at high risk or lost to care.
- To help engage individuals at high risk or lost to care, many organizations require additional resources to manage peer outreach counselors, small stipends for outreach workers, enhances computer systems for data tracking, and more. ASOs will be required to monitor increased engagement in care, defined by the number of HIV-positive individuals at high risk or lost to care who have:
- Returned for two or more medical appointments
- Achieved an undetectable viral load
- These indicators will track performance towards achievement of the overall goal of reduced community viral load and improved health outcomes for HIV-positive individuals in Pennsylvania. Due to the importance and potential of this initiative, Subgrantees will be expected to fully track and submit data related to the program.
- To help engage individuals at high risk or lost to care, many organizations require additional resources to manage peer outreach counselors, small stipends for outreach workers, enhances computer systems for data tracking, and more. ASOs will be required to monitor increased engagement in care, defined by the number of HIV-positive individuals at high risk or lost to care who have:
- Grant awards to AIDS Service Organizations (ASOs) to continue programs to identify and enroll eligible clients in the Special Pharmaceutical Benefits Program (SPBP).
- Uninterrupted access to treatment is a key component in achieving an undetectable viral load and improved health outcomes. This program aims to improve access and adherence to treatment through proactive identification and enrollment of SPBP eligible clients in the program.
- ASOs will be required to monitor identification of SPBP eligible clients and enrollment in SPBP throughout the duration of the grant. The project aim is that 80% of eligible clients are enrolled in the SPBP.
- Utilize Quality improvement tools to develop strategies and plans to sustain agency efforts to engage lost-to-care and high-risk clients in medical care.
- Utilizing the tools for quality improvement, such as workflow redesign and defining standard work, agencies will identify opportunities to improve efficiency and quality of care and sustain best practices and models of care for engaging clients who are high risk or lost to care within their agencies. In order to support agencies in their sustainability efforts, the Lead Agency will continue to facilitate Minority AIDS Initiative RFP 2022-2024
collaboration among Subgrantees to share best practices and tools as well as provide limited coaching support.
- Utilizing the tools for quality improvement, such as workflow redesign and defining standard work, agencies will identify opportunities to improve efficiency and quality of care and sustain best practices and models of care for engaging clients who are high risk or lost to care within their agencies. In order to support agencies in their sustainability efforts, the Lead Agency will continue to facilitate Minority AIDS Initiative RFP 2022-2024
- Documentation of successful outcomes, implementation challenges, and lessons learned.
- With the support of the Lead Agency, Subgrantees will be asked to participate in sharing and documentation of lessons learned during program implementation. The content for these lessons will be brought together from multiple sources, including data collection, case studies, interviews, and more, so that success and learning across a number of aspects of this initiative’s model will be clearly understood and articulated. This information will be collected throughout the Subgrant period in order to capture learning in real-time, and to minimize reporting requirements at the Subgrants conclusion. Opportunities for networking and shared learning across organizations are encouraged, and the Lead Agency will facilitate specific opportunities throughout the Subgrant period.

- Funding is available to Subgrantees from July 1, 2022 to June 30, 2024 for the Minority AIDS Initiative.
Eligibility Criteria
- Applicant must either be a not-for-profit 501(c)(3) organization, or a for-profit organization able to provide documentation that the Applicant is the only available provider of quality HIV care in the area. All for-profit proposals require prior approval from the Pennsylvania Department of Health.
- Applicant can have no record of unsatisfactory performance. Applicant must not be presently debarred, suspended, proposed for debarment, or declared ineligible or voluntarily excluded from participation in this transaction by any local, state, or federal department or agency.
- Applicant must have the ability to maintain adequate files and records to meet reporting requirements. This includes having reliable computer systems with the capacity to host CAREWare.
- Applicant must have the administrative and fiscal capability to provide and manage the proposed services on a reimbursement basis and ensure that adequate documentation related to the services can be provided.
- Applicant must demonstrate the capability to perform all elements of the proposed scope of work and have the capacity to enter a Subgrant Agreement with JHF.
- Applicant must possess the appropriate licenses and certifications issued by the Commonwealth of Pennsylvania, if required based on the services proposed.
- Applicant must be legally authorized to conduct business in Pennsylvania and must have established administrative and programmatic resources to provide services within the 11- county region of Southwestern Pennsylvania.
