Eligibility Criteria
- Applicant must be a nonprofit, 501 (c) 3 tax-exempt organization; a school-based health program; or have a 501 (c) 3, tax-exempt organization as a fiscal sponsor.
- The request must align with one or more of the strategic focus areas: Building Health Equity: An equal and fair opportunity for every individual to attain the highest level of health and wellness regardless of socioeconomic barriers, and/or Increasing Wellness in Pajaro Valley: The active pursuit of activities, choices, and lifestyles that emphasize preventing illness and improving quality of life, as opposed to emphasizing disease treatment.
- The request should serve individuals living in the Pajaro Valley.
In general:
- This grant is intended to support program activities conducted within the Pajaro Valley. Applicants must clearly demonstrate how their proposed project aligns with this geographic focus in their application.
- Organizations that have been funded for three consecutive years are welcome to apply, priority will be given to newer requests.
- Community Health Trust’s Board prefers not to fund programs or projects administered by government agencies, with the exception of school-based health programs closely aligned with the CHT’s mission.
