The Howard County Health Department (HCHD) is accepting applications for its Health Disparities Grants Program to engage community organizations to reduce health disparities related to chronic disease and COVID-19 infections in identified Howard County communities.

The Local Health Department Health Disparities Funding Opportunity has allowed the Health Department to fund two categories of grants. The first is for local organizations to identify and recruit community leaders to create a network of certified Community Health Workers (CHW) to aid in chronic disease prevention and management, and to decrease hospitalization utilization rates in identified communities. The second grant will be awarded to local organizations to provide ongoing support for this CHW network.

Chronic diseases considered were diabetes, hypertension and heart disease, and chronic kidney disease. Using data maps to identify areas in the county with greater health disparities, HCHD seeks to partner with community organizations to reduce health disparities in the following zip codes: 21044 and 21045 (Columbia), 20794 (Jessup), 20763 (Savage) and 20723 (Laurel).

HCHD plans to use these funds to create a network of Community Health Workers (CHWs) in the selected zip codes. These CHWs will provide resource referrals and basic health information to residents. HCHD will provide funding for CHWs to become certified through Howard County General Hospital’s state-accredited CHW certification program.

Project must address one of the following grant objectives:

  • Identify and recruit community leaders from identified communities to become certified Community Health Workers
  • Support a network of Community Health Workers who are certified or plan to become certified to provide referrals and outreach in identified communities.

Funding Information

  • Category 1: Proposals in this category shall not exceed $5,000
    Category 2: Proposals in this category will be paid $4,913 per CHW for the year and prorated by the month.
  • The funding period for this grant is as follows:
    • Category 1: September 1, 2022 – January 15, 2023
    • Category 2: September 1, 2022 – June 30, 2023

Eligibility Criteria

Eligible proposals must fulfill one of two categories. Organizations applying for both categories must submit separate applications. Organizations can identify a sub-award grantee to manage a portion of the tasks, but one organization must be identified as the prime.

  • Category 1: Identify and recruit interested candidates to become Community Health Workers or residents who are already certified Community Health Workers and who live in zip codes 21044, 21045, 20794, 20763 and 20723.
  • Category 2: Provide support for certified Community Health Workers and Community Health Worker candidates in one or more of the selected zip codes (21044, 21045, 20794, 20763 and 20723).
  • Applicants must select Category 1 or 2. If applicants are applying for both categories, separate
    applications are required.

    • Applicants must meet the following eligibility criteria:
      • A current non-profit and
      • Registered and in ”Good Standing” with the State.
