Texas Health Community Impact (THCI) Collin Leadership Council is requesting proposals from community-based organizations (CBOs) to address health access, literacy, and navigation as identified by the Texas Health Community Health Needs Assessment (CHNA).

Details:

Collin County is a quickly growing county with diverse demographics and an ever-increasing income inequality gap. Coupled with social barriers, such as the lack of transportation, limited access to affordable and healthcare and affordable housing, Collin County continues to face challenges that are specific to its demographic and geographic makeup, challenges which are distinctly different from those of other North Texas regions. With the population growth, and the lack of transportation and affordable housing, gaps in services will only widen. THCI Collin Leadership Council is eager to maximize innovative proposals to achieve its goal of increasing health equity in underserved populations in the targeted ZIP codes. Innovation and creative practices will drive the focus and resources. By supporting innovative strategies and sharing lessons learned across region, state, and nation, the THCI Collin Leadership Council intends to contribute promising practices to improve public health.

Program Goal

Increase health equity in the underserved populations by expanding healthcare access and reducing social barriers (food insecurity, transportation, and access to culturally competent services) in 75074, 75069, 75442 and 75407.

Program Objectives

There are three (3) eligible program objectives, and applicants are encouraged to submit a proposal that includes all three (3) program objectives. Demonstrated collaborators with partners sharing responsibilities to carry out the full scope of objectives as described below will be given greater consideration. Alternatively, organizations may also submit proposals that do not incorporates all three for consideration.

  • Objective 1.1: Increase access to quality care by creating innovative patient centered solutions.
  • Objective 1.2: Increase the ability of behavioral health professionals to provide high quality care to patients.
  • Objective 1.3: Create an innovative solution to address social barriers (i.e., lack of access to food, transportation, culturally competent services) preventing optimal health.

Funding Information

  • Total amount available for all of Collin Region is $1.2 million.

Eligibility Criteria

The following is a non-exclusive list of potential lead organizations that may be eligible to apply:

  • State, county, city, or township governments
  • Independent school districts
  • Public or state-controlled institutions of higher education
  • Public housing authorities
  • Nonprofits having a 501(c)(3) status with the IRS
  • Hospital systems can be a part of a collaboration but cannot serve as the lead applicant.
