OPHE has committed $2.4 million ($1.2 million per year) to building the capacity of Population Health Equity Collectives (Collectives). These Collectives will address the needs of six (6) targeted and prioritized populations:
- Asian American, Native Hawaiian, and Pacific Islander community
- Black or African American community
- Latino, Hispanic, Chicano, or Latin American community
- Older adult community (65+)
- Lesbian, gay, bisexual, transgender, and queer (LGBTQ+) community
- Individuals with disabilities
The overarching goals of these Collectives are to:
- Address health inequities and improve social determinants of health through collaboration, partnership, and inclusion of lived experiences and authentic voices
- Overcome inequitable access to opportunities, resources, and support services for targeted and prioritized populations in Orange County
- Build and support cohesive and sustainable Collectives in Orange County and to address systemic health inequities facing prioritized impacted communities
- Provide venues for community voice in identifying and determining solutions and priorities in addressing health inequities of the Collective
Funding Information
The Equity in OC Initiative will provide funding of $200,000 per year to six (6) Collectives to build their internal capacity as a Collective to improve health equity of their respective communities in Orange County.
Eligibility Criteria
To be eligible for funding, a Collective must include 5 or more organizations (one anchor/backbone organization and at least four partner organizations) that meet all of the following criteria:
- All organizations within the Collective must be based in Orange County and provide programs or services to Orange County residents.
- As a Collective, the organizations should be primarily focused on serving one of the six specific populations for which they are applying. In particular, a minimum of 50% of each organization’s clients must be from that identified population (Black, Latino, Asian and Pacific Islander, LGBTQ+, individuals with disabilities, or older adults).
- Collaborate with other organizations to form new or foster existing and expanded relationships:
- New – new partnerships include any organization that the recipient has not worked with in the past in funded or unfunded capacities
- Existing – existing partnerships include organizations that recipients are currently working with or have worked with previously to address health disparities and inequities
- Expanded – expanded partnerships include those that recipients are currently working with or have worked with previously and will enhance through increased membership, mission, or funding
- Build the capacity of the Collective as a whole to address health inequity in Orange County, including COVID-19 health-related disparities.
- Must not discriminate based upon age, gender, gender identity, race, disability, religion, national origin, sexual orientation, or an individual’s ability to pay.
- Identify one anchor/backbone organization for the Collective, who commits to:
- Having 501(c)(3) tax-exempt status
- Receive up to 15% of the budget (maximum $30,000 annually) for grant administration and implementation and to oversee evaluation, reporting, and other requirements
- Support the development of leaders who will generate and contribute to solutions that impact all levels of the community
- Prioritize the unlocking of untapped potential in people and organizations as a pathway to abundance
- Invest in a change process that is dynamic and enhances multi-sector engagement, relationships, capacity, and intention for change
