Minnesota Department of Health announces Health Care Transition Learning Collaborative Grant

The Minnesota Department of Health’s (MDH) Children and Youth with Special Health Needs Section (CYSHN) is seeking proposals from qualified organizations to facilitate a Health Care Transition Learning Collaborative aimed at improving transition from pediatric to adult health care settings for youth with special health needs and disabilities (YSHND).

Details:

The purpose of the grant is to improve the competence and confidence of health care providers in managing the care of youth and young adults with special health needs and disabilities through the health care transition process in a culturally responsive, trauma-informed manner. The grant will bring together subject matter experts, health care providers, YSHND and their families, and other systems partners to reduce silos and build infrastructure for a model of care delivery that ensures YSHND are able to successfully integrate into adult systems of care.

Preference will be given applicants who select an evidence-based tool/structure for implementing the Learning Collaborative, such as the Project ECHO® (Extension for Community Healthcare Outcomes) model.

Funding Information

Funding will be allocated through a competitive process. If selected, you may only incur eligible expenditures when the grant agreement is fully executed, and the grant has reached its effective date.

A total of $160,000 in state funds is available to fund one applicant for the grant period of May 1, 2023, through June 30, 2024.

Eligible Applicants

Eligible applicants can include community-based nonprofit and for-profit organizations; government entities or Tribal Governments; clinics and hospitals; community health or public health organizations; quality improvement organizations; professional provider associations; and institutes of higher education.

Desired skills of applicant include:

  • Subject matter expertise around the topics of health care transition, change management, quality improvement, etc., and/or existing relationships with individuals with subject matter expertise.
  • Knowledge of principles of adult learning and experience designing and implementing curricula for a variety of audiences.
  • Ability to effectively involve patient/family partners in curriculum development, learning sessions, evaluation, and sharing feedback.
  • Demonstrated ability to implement face-to-face, virtual, and distance learning activities in a collaborative manner.
  • Experience with evaluating the process and outcomes of learning collaborative activities.
