Grants for fiscal year 2022 not to exceed $25,000.00.
Eligibility Criteria
Grants for programs or services shall be considered based on the following criteria:
- Organizations must hold not-for-profit, tax-exempt status, 501©(3)
- Projects should contribute to improving the overall health status of the community including social determinants of health, for example:
- Education (ex. Literacy programs, higher education programs, health education programs)
- Employment (ex. Soft skills training, apprenticeship programs, internship programs)
- Health (ex. Quality care access, healthy food access, screening, behavior health access)
- Safety (ex. Transportation, housing, green space access, violence education and prevention)
- Projects should be used to establish new programs or support existing initiatives that improve health and wellness.
- Projects should go to support marginalized communities.
- Projects should foster collaboration between organizations.
- Incorporates population health & community health improvement principles relating to access, service, detection, prevention, disease management, safety outcomes and financial measures.
- Grant allocation one year does not guarantee funding yearly.
