Details:

Community health investment grants are allocated to non-profit organizations for specific programs and services that address identified community health needs. To qualify, activities must seek to provide measurable improvement in health status, access, or use of healthcare resources and must serve individuals in the Owensboro Health service area.

Grants are also awarded to non-profit organizations for specific programs and activities not directly related to healthcare but address root causes of health problems that impact the health of  community. All activities must serve individuals in the Owensboro Health service area.

Funding Information

Organizations that provide evidence of a 100-percent tobacco-free policy may apply for up to $1,000 for a health project and up to $500 for an event. Total allocations for the fiscal year may not exceed $1,000.

Eligibility Criteria

  • 501(c)3 organizations, government and public entities are eligible to apply.
  • Grants to Section 501(c)(4) organizations may be considered if the proceeds are used exclusively for section 501(c)(3) permissible purposes, are not used for lobbying, and are not used for political campaign activities. A 501(c)(4) recipient must report back to Owensboro Health on its use of the grant proceeds. Further, grants to 501(c)(4) organizations will not be permitted to cover the recipient’s fundraising costs or to provide general support. A 501(c)4 recipient, like all grant recipients, must provide a mid-year and end of year report to Owensboro Health on its use of grant proceeds.
  • Organizations cannot apply to both Owensboro Health Regional Hospital and Owensboro Health Muhlenberg Community Hospital mini-grant programs. Organizations in Muhlenberg County must apply for Owensboro Health Muhlenberg Community Hospital mini-grant.
  • Requests for event funding may only be made for events from which proceeds raised are used to impact community health. If considering a request for event sponsorship, organizations are advised to submit an explanation of how funds raised from their event will be used for the projects, programs and activities in the Owensboro Health service area focused on impacting health indicators.
  • Programs for which funds are requested must focus on providing activities or services that address an identified health need or that address root causes of health problems (if not directly related to healthcare). The connection between health needs and the activity or service must be clearly stated on the application.
  • All requests must include a project/fundraising budget.
  • Event organizer must obtain necessary permits, licenses or insurance and provide verification of such to Owensboro Health if requested.
  • Owensboro Health shall not be financially or otherwise liable for promotion staging of events.
  • Geographic area: Services supplied by requesting organizations must impact citizens within the Owensboro Health service area (Breckenridge, Butler, Daviess, Edmonson, Grayson, Hancock, Hardin, Hart, Henderson, Hopkins, Logan, McLean, Muhlenberg, Ohio, Union and Webster counties in Kentucky and Spencer and Perry counties in Indiana).
  • If an organization wishes to recognize Owensboro Health as part of an event, an explanation in writing of your plan to do so must accompany the application prior to the event.
