The 2022 Community Health Needs Assessment identified healthcare access and quality of care, behavioral health, and social determinants of health as areas for improvement in Collin County. The Collin Leadership Council is seeking innovative proposals to increase health equity in the underserved populations by expanding healthcare access, increasing behavioral health services, and reducing social barriers such as food insecurity, transportation, and access to culturally competent services, in East Plano (75074), East McKinney (75069), Farmersville (75442) and Princeton (75407). The strategic plan includes program-specific and population focused approaches to maximize resources and enhance the impact of resources in these high-need areas of Collin County.
Priority Health Issue
THCI strives to advance our commitments and fulfill our mission and vision by focusing on the essential issues in Collin County. The foundational goal of our strategic plan is to increase access to healthcare, including behavioral health, and address social determinants of health through innovative and collaborative community partner projects. According to the Agency for Healthcare Research and Quality, healthcare access can be defined as “the timely use of personal health services to achieve the best health outcomes” and consists of four primary components :
- Coverage: Allows basic entry into the healthcare system, particularly for uninsured individuals who are less likely to receive medical care and more likely to have poor health outcomes.
- Services: Having a usual source of care, particularly for recommended health screenings and prevention services.
- Timeliness: The ability to receive healthcare services when needed.
- Workforce: Highly qualified and culturally competent healthcare providers.
Proposed projects should:
- Focus on reducing the negative impact of social determinants of health for the underserved.
- Incorporate innovative solutions to improve health equity.
- Have a solid implementation plan that considers cultural sensitivities and addresses community needs.
- Clearly demonstrate the level of involvement and contributions from collaborators, including deliverables and budgetary commitments.
- Identify opportunities for Texas Health to engage through volunteering, education, or other.

Eligibility Criteria
The following is a non-exclusive list of potential lead organizations that may be eligible to apply:
- State, county, city, or township governments
- Independent school districts
- Public or state-controlled institutions of higher education
- Public housing authorities
- Nonprofits having a 501(c)(3) status with the IRS
- Hospital systems can be a part of a collaboration but cannot serve as the lead applicant.
