Eligibility Criteria
- Must be a nonprofit that addresses one of the four critical needs in the community – education, financial stability, health and basic needs.
- Have a current 501 (C)(3) tax status
- Been in operation for at least one year
- File an applicable Form 990 with the IRS annually
- Conduct an annual audit by an independent CPA or provide other detailed financial statements (determined based on annual revenue of agency)
- Maintain an active rotating Board of Directors
- Has an established Charter of Incorporation and By-Laws
- Obtains a current license and accreditation to deliver services (if applicable)
- Offers a sliding fee-scale policy (if applicable)
- Maintains a board adopted policy of non-discrimination for hiring and service delivery
- Produces an Annual Report
