Preferential treatment for collaborating organizations will be considered in the review process. Please note that information on each of the organizations will be requested in the application.
  • Special Healthcare Initiative – Requests related to the Foundation’s Special Healthcare Initiative for nonprofit organizations demonstrating a plan that focuses on collaboration and innovation to improve population health. Applicants must document the need(s) that will be addressed, the population size to be served, how results will be measured and the 3-4 year plan for the project. Note that this initiative is only available to groups in selected counties in New York State (Clinton, Essex, Franklin, Hamilton, Montgomery, Warren and Washington counties).
