Developed in partnership with members of Cobb2020 and Live Healthy Douglas, the CHIP asks two key questions as strategic priorities for community health improvement:
- Access to Health Services – How do they provide ample, accessible, culturally competent, and quality health services to meet the needs of the diverse community?
- Basic Needs and Healthy Lifestyles – How do they create a community that satisfies its members’ basic needs and provides the conditions necessary to achieve overall wellbeing?
For each strategic priority, the CHIP identifies goal areas of focus with corresponding strategies to achieve the goals. The goal areas are:
- access to health care;
- healthy behaviors (e.g. physical activity, healthy sexual behaviors, worksite wellness programs, chronic disease prevention programs, reducing youth tobacco/vape use, and promoting cessation among adult tobacco users);
- healthy eating;
- maternal and infant health; and
- mental and behavioral health.
Funding Information
The CHIP Partnership Grants offer up to $25,000 per agency through $1:$1 matching funds. Requests less than or equal to $5,000 do not require a match. The proposed projects must be new or expanded efforts to address the strategic goal areas of the CHIP, require partnerships with other organizations, and must include policy development activities, system-level improvements or built environmental changes that will support long-term and sustainable progress.
Grant Period
October 15, 2025 to June 15, 2026.
Criteria
Eligibility is open to any type of organization; however, the proposal should not be intended to generate a profit. Any program income earned because of the grant must be used for approved project-related activities.
Priority will be given to applicants who:
- Have not been awarded funding in previous years;
- Demonstrate sustainability beyond the funding period;
- Have demonstrated reliable and consistent reporting and accountability for grant funds; and
- Utilize evidence-based and best practice community health initiatives.
