- A copy of the 501(c)(3) IRS letter of determination if the applicant is a non-profit organization (Note: an IRS (c)(6) must partner with a (c)(3) organization or government entity and the application will be submitted from one of those organizations)
- Most recently submitted IRS Form 990 (for non-profits only)
- Project narrative of no more than two pages
- A budget for the project or program that includes in-kind or contributions from other sources and the total cost.
