- Have filed a Form 990 tax return with the IRS within the last year
- Have audited financial statements by an independent accountant within the last eighteen (18) months
- Be headquartered in Arizona, with the majority of staff, services provided, and requested funding in Arizona
- Have an annual budget of at least $500,000 and no more than $5 million
- Not have received funding from the OneAZ Community Foundation in the previous year. If funding was received in the previous year, you are not eligible to apply in the current year
- Be mission-aligned with the OneAZ Community Foundation’s funding pillars
