CareOregon will consider community grant requests for support of programs and services as described below.
- Food & Nutrition Education programs: teaching kitchen and community garden programs that promote health and wellness, in addition to workforce development programming
- Housing Support services: especially resident services, targeted eviction prevention support and houseless community member support
- Access to Care & Navigation services: culturally specific healthcare navigation services, promotion of health literacy, and primary care engagement.

Eligibility Criteria
- CareOregon considers requests from tax-exempt, community-based organizations within its immediate service area of Multnomah, Washington, Clackamas, Columbia, Clatsop, Tillamook and Jackson counties in Oregon. Organizations based in other counties must have services within CareOregon’s immediate service area to be considered for community grant support.
- CareOregon seeks to support programs that:
- Impact CareOregon or Oregon Health Plan (OHP) members
- Address priority social needs in the communities where CareOregon or OHP members live
- Build CareOregon partnership with community-based organizations to address social needs in the community
- Increase collaboration between community-based organization partners to improve access to care and reduce health disparities.
