The Chatham County Hospital Authority (CCHA) Trust is offering a $2 million grant opportunity for organizations providing preventative health care services to indigent residents in Chatham County, Georgia. Both nonprofit and for-profit organizations are encouraged to apply through a competitive grantmaking process.
The grant aims to support preventative health care for individuals and families living at or below 138% of the Federal Poverty Line in Chatham County. United Way of the Coastal Empire will administer the grant process, though CCHA retains full decision-making authority over awards.
Applicants can request up to $250,000. Eligible projects must address one or more focus areas: behavioral and mental health services, health screenings, dental hygiene care, or specialty care such as diabetes management. Organizations must serve indigent Chatham County residents and provide documentation verifying client income and residency.
Eligible applicants include 501(c)(3) nonprofits or for-profit businesses with proper licenses and certifications. Proof of insurance, such as nonprofit liability or for-profit malpractice insurance, is required. Organizations must comply with federal and state anti-discrimination laws and the USA Patriot Act.
For-profit applicants must submit recent tax returns, income statements, balance sheets, and audits or reviews. Nonprofits must provide similar financial documents. All applicants must meet CCHA eligibility standards, have a conflict-of-interest policy, and cannot use funds for capital campaigns or fundraising events.
Grantees will be required to submit mid-year and final reports detailing data and outcomes as a condition of funding.
