Details:

The Governor’s Office of New Opportunities for Rural Transformational Health (GO-NORTH) is leading New Hampshire’s commitment to making rural health care more connected, resilient, and responsive
through transformational change across the state’s rural health system. Under GO-NORTH’s direction, FHC has been selected as a Hub implementing organization and is responsible for administering GONORTH investments to strengthen rural health service delivery and expand access to affordable, high-quality care. Through GO-NORTH, FHC will award competitive grants to rural health providers and organizations committed to sustainable health care innovation and long-term improvement in partnership with the communities they serve.

Funding Information

  • Stabilize & Strengthen Rural EMS Systems: Up to $25,000
  • MIH Expansion of Existing Programs: $100,000–$400,000
  • MIH New Regional Programs: $300,000–$2,000,000

Grant Period

September 30, 2027 to September 30, 2031.

Uses of Funds

General Allowable Cost Categories:

  • Personnel costs related to implementation
    • Data collection and evaluation
    • Quality improvement
    • Building regional partnerships
    • Developing processes, policies, and workflows
    • Health information technology support
    • Engagement with health systems and payers/funders
    • Sustainability planning including building a regional approach and coordinating care
    • Mentoring and training staff, time spent in training and meetings with partners
    • MIH service delivery to 2 and 3
  • Training/continuing education for existing staff
  • Consultants
  • Travel – out of state for professional development directly related to the project, in-state meetings Technology investments (communication systems, remote monitoring tools, data systems, interoperability)
  • Regional collaboration and shared service planning
  • Funding for other operational associated costs for MIH startup
  • Cost assisting with data and quality reporting

Eligibility Criteria

All New Hampshire-based nonprofit, for-profit, or community-based organizations are eligible to apply. Nonprofits must be in good standing with the NH secretary of state. Applicants must fit into one of the following categories:

  • Licensed NH EMS Units
    • Organizations without EMS licensure but have licensed NH EMS Unit formally involved in the application
    • Organizations in pursuit of becoming a NH licensed EMS Unit (must submit appropriate documentation) It is encouraged that multiple eligible organizations partner on one application, with one organization designated as the lead applicant.
  • The following entities are ineligible to apply:
    • Individuals
    • Organizations with existing legal proceedings involving the State of New Hampshire (or any agency or subdivision thereof) or any other state/federal government entity before and adjudicative body in any jurisdiction.
