Funding Information

Grant-funded programming must align with Trinity Health’s Mission, Vision and Values, and must address at least one of the top 5 priorities of the most recent Kent County Community Health Needs Assessment (CHNA):

- Access to Medical Care
- Access to Healthy Food
- Mental Health.
