The CCME Foundation works to improve the health of individuals and communities.
Foundation prioritizes projects that focus on measurable outcome improvements and have a high impact on health care improvements and access, particularly for uninsured and underserved populations.
A focus on improved outcomes and access to health care is essential. In a constantly evolving health care landscape, this often requires new approaches and innovations. The CCME Foundation is implementing its Promising Practices Framework to encourage partnership and innovation at the grassroots level primarily in rural and sparsely suburban populations and/or in under and unserved populations within the following priority focus areas: post-acute care, behavioral health, chronic disease management, and workforce development.
Funding Information
Grants range from $5,000 to 50,000 per organization.
Grant Period
The grant year begins on August 1st of each year and ends on July 31st of each year.
Eligibility Criteria
Applicant organizations must operate within North Carolina and/or South Carolina. All organizations with current 501(c)(3) status focused on improving the health of individuals through measurable outcome goals at the community level are encouraged to apply. Examples of organizations that may apply for funding are:
- Faith-Based Entities
- Hospitals/Providers/Clinics
- Educational Institutions
- Community-Based Organizations (CBOs).
