The Advancing Strategies to Support Older Adults with Behavioral Health Conditions in the Senior Nutrition Program is designed to support a project for behavioral health-focused innovations in the senior nutrition program setting with intentions to 1) learn of existing barriers and facilitators to serving older adults living with behavioral health conditions in the congregate meal setting, 2) develop and implement person-centered, trauma-informed, and appropriate best practices to meet the needs of seniors living with behavioral health conditions in the senior nutrition program setting, and 3) develop and disseminate resources from tested findings and already existing practices.
This NOFO has the following goals: 1) learn of existing barriers and facilitators to serving older adults living with behavioral health conditions in the congregate meal setting, 2) based on learnings from Goal 1, train staff and sub-grantees to develop and implement person-centered, trauma-informed, and culturally appropriate best practices to meet the needs of older adults living with behavioral health conditions in the congregate nutrition program setting, 3) evaluate the effectiveness of implemented practices through measurable outcomes, and 4) create and share a minimum of ten (10) resource materials and disseminate findings to help the senior nutrition program network adopt best practices.
