By offering general operating support grants, HealthPath aims to enable organizations to allocate resources where they are most needed, fostering innovation, impact, and sustainable systems change that promotes health equity in Ohio’s communities.
Funding Priorities
- Oral Health: Ensuring access to essential dental services for overall health and well-being, particularly for underserved populations
- Age-Friendly Communities: Creating communities that enable older residents to live, keep connected, and stay engaged in their communities and contribute to their community’s economic, social, and cultural life by considering the environmental, economic, and social factors that influence health and well-being as they age
- Healthy Ohioans: Expanding access to fundamental health resources, including healthcare and nutritious food.
Funding Information
- Organizations may apply for grants of up to $25,000
- HealthPath expects to award up to $250,000 in total funding.
Populations of Focus
Proposals should also prioritize one or more of the following populations in Ohio, which are disproportionately affected by health inequities:
- Black, Indigenous, and People of Color (BIPOC)
- Individuals with lower levels of education and/or income
- Individuals living with disabilities
- Adults aged 60 or older
- Immigrants and new Americans
- Individuals living in rural or Appalachian communities
- Individuals lacking adequate health insurance coverage
- Individuals who identify as LGBTQIA+.
Eligibility Criteria
- Align with HealthPath’s mission and vision
- Be a 501(c)(3) nonprofit or government organization located in and primarily serving at least one of the 36 counties in HealthPath’s service area
- Address health and/or social needs that impact health outcomes
- Demonstrate alignment with the funding priorities and populations of focus outlined in the RFP
- Not have an active grant through HealthPath
- Organizations without 501(c)(3) status may apply through a fiscal sponsor. Fiscal sponsorship will be reviewed on a case-by-case basis. Groups interested in this option should contact Abby Haynes before applying to ensure eligibility.
