The purpose of the Inova Community Health Opportunities is to improve the health of the communities described in the Inova Community Health Needs Assessments (CHNA). The objectives of the proposed project should be clear, reasonable, and attainable, with measurable impact on the addressed need(s) related to the selected health issue(s).
The goal of the Inova Community Health Opportunities is to fund a diverse set of proposals that collectively:
- Reach all CHNA regions
- Serve communities with identified needs
- Cover all CHNA priority health issues
- Address most/all Social Drivers of Health.

Eligibility Criteria
Inova is dedicated to supporting projects that make a difference in the quality of life, health and welfare of the communities it serves. Inova recognizes the collective impact of community partners to create positive social impact, however is limited in the number of projects that it can support. An organization with interest in submitting an application must, at a minimum, meet the following criteria:
- Not a recipient of the 2025 Inova Community Health Opportunities Funding
- Tax-exempt under Section 501(c)(3) of the Internal Revenue Service Code
- Located in and serve residents in one of the areas included in the Inova CHNAs
- Address one of the top health issues identified on pages 19-23 of the relevant regional CHNA report(s).
