The Linkages to Care (LinC) funding opportunity seeks to strengthen broad-based, multi-stakeholder networks and partnerships between community-based organizations (CBOs) and healthcare entities.

Details:

The goals of the program are:

  • to support people who use substances and those seeking treatment by building connections and collaboration across systems and disciplines, and
  • to reduce overdose deaths by promoting safety, wellness, and recovery by eliminating barriers and increasing coordination during points of transition.

This funding opportunity promotes broad-based, multi-stakeholder networks between hospitals, inpatient treatment providers, outpatient treatment providers, CBOs, clients and their families, and other key stakeholders to improve linkages to social needs, community, and recovery supports. This will help to strengthen linkages between levels or types of care by utilizing engagement strategies, removing barriers to access, and fostering shared care management through a network of partners consisting of treatment, recovery, and harm reduction organizations and healthcare providers.

Funds can be used to support a range of items, including but not limited to:

  • Staffing and salaries, education materials, data systems, staff education, translation, health education and group activities, transportation, triage activities, waitlist management, and similar infrastructure enhancements.
  • Peer support or community-based follow-up and engagement strategies to facilitate connections to treatment and wraparound services in coordination with individualized treatment plans or services requested by the client.
  • Convening partners regularly to build relationships, strengthen collaboration, and promote strong linkages to care across the continuum.
  • Activities that explicitly address health disparities and the health outcomes of populations that have been historically underserved.

Funding Information

RIZE is offering a limited number of grants of up to $125,000 per year for two years to fund the development of a comprehensive LinC program.

Eligibility Criteria

RIZE invites applications from non-profit community-based treatment, harm reduction, and recovery programs in Massachusetts. Selected grantees should be able to demonstrate an existing or new partnership with a hospital, community health center, or other healthcare provider that provides inpatient treatment services through a letter of support or a memorandum of understanding.
