Funding Information
- Estimated Total Program Funding: $3.75 million
- Anticipated Individual Award: $50,000-$250,000 in base awards (depending on the number of counties applied for and awarded).
Grant Period
Award recipients are expected to expend the award amount no later than October 30, 2026, and submit the invoice(s) by November 30, 2026, to OSDH.
Eligibility Criteria
- Applicants are required to meet all the criteria below in order to be eligible for a microgrant.
- Be currently providing services in Oklahoma.
- Be located in a Rural Oklahoma County – or have a branch in a Rural County where said branch is to be the recipient of the microgrant investment. Rural Counties are defined as all counties except for Tulsa County and Oklahoma County.
- Provide services to communities with populations of 55,000 or fewer residents.
- Provide services that promote the public’s health.
- Applicants must have an active Unique Entity Identifier (UEI) and maintain an active registration in SAM.gov throughout the application, review, and award period. Applicants must not be suspended, debarred, or otherwise excluded from participating in federally funded programs or activities.
- Applicants must have an active Certificate of Insurance.
