Jackson Care Connect is now accepting applications for its 2025 Community Health Improvement Plan Grant. This grant aims to support and improve the health quality of individuals within the community. In 2024, Jackson Care Connect collaborated with community partners and the Jefferson Regional Health Alliance to conduct a Community Health Assessment and develop a Community Health Improvement Plan.
The grant focuses on several priority areas. These include improving access to healthcare by addressing barriers, ensuring timely care, and promoting trauma-informed, culturally appropriate, and patient-centered services. Behavioral health is another priority, aiming to reduce negative outcomes related to mental health and substance use disorders while increasing access to behavioral health services and crisis response systems.
Community building is emphasized to boost social connections that are inclusive and culturally relevant. It also encourages diverse community representation in leadership and decision-making for health initiatives. Additionally, the grant targets housing, seeking to increase emergency and affordable housing options, reduce homelessness through outreach, and sustain dignified housing choices for the unstably housed.
Applicants can request up to $20,000 per grant. Eligible applicants include nonprofit organizations and government agencies. Applications must address at least one of the CHIP priority areas: Access to Health Care, Behavioral Health, Community Building, or Housing. Grant applications will be reviewed by the Community Advisory Committee along with other community organizations and Jackson Care Connect staff. The deadline for submissions is November 30, 2025.
