Partner with the Urban American Indian Community and Organizations Program (Minnesota)

The Minnesota Department of Human Services, through its Transitions, Tribal & Transformation Division (STATE), is seeking proposals from qualified Responders to Partner with the Urban American Indian community and organizations to identify needed services, key priorities, and barriers that impact access to Home and Community Based Services.

Details:

The Urban AI/AN Elder home and community-based services (HCBS) Expansion Initiative will support DHS’s work to strengthen and expand access to culturally responsive HCBS for American Indian and Alaskan Natives in the Urban American Indian communities in Minneapolis and Saint Paul.

The initiative focuses on American Indian adults aged 55 and older who are receiving, or may be eligible for, HCBS. Through this project, DHS aims to identify barriers, strengthen culturally relevant supports, and create strategies that make HCBS more accessible for elders in the Urban Indian community.

Tasks and Deliverables

Responders must address the following tasks and deliverables within their proposals. This section is the core of the RFP: focus on the outcomes and deliverables of the project.

  • Partner with Urban AI/AN provider organizations who offer culturally relevant services to identify and reach elders who may qualify for HCBS;
  • Conduct interviews and/or talking circles with eligible individuals and staff from Native-serving organizations to gather insights;
  • Develop and implement a community engagement plan grounded in cultural practices and respectful relationship-building;
  • Create a landscape assessment and summary report estimating how many people may be eligible for HCBS Include recommendations based on the findings;
  • Identify needed services, key priorities, and barriers that impact access to HCBS. Determine what HCBS services are available to native elders living in the urban area; develop culturally specific recommendations tied to HCBS and integrated into long-term planning; and
  • Co-design recommendations with DHS and deliver findings in a formal summary or report to inform future policy and program development.
    • Findings and recommendations will be presented to both DHS and Community Providers.
    • Written summary of the findings and recommendations will be provided to DHS.

Funding Information

  • A total of two hundred forty-seven thousand and two hundred forty-seven dollars ($247,247.00) is available for award; subject to available funds.
  • The term of any resulting contract is anticipated to be for one year, from July 1, 2026 until June 30, 2027. STATE may extend the contract up to a total of five (5) years.
