The goal of the Community Health Program is to reduce exposure to or alleviate the health impacts associated with port-related air pollution by supporting health programs that serve children, pregnant women, senior citizens, the chronically ill, and people with respiratory/cardiopulmonary illnesses and impairments.

Up to 3 years
Eligible Projects
The following projects are eligible for funding:
- Health projects and services designed to reduce the risks associated with asthma and/or other cardiopulmonary ailments. These projects may include services such as screening and diagnosis, case management, education, relevant training, and outreach.
Allowable Expenses
Costs eligible for funding under the Grant Program are listed below.
- Equipment, supplies, diagnostic equipment, test kits and installation costs;
- Staff training related to the project goals;
- Computer software;
- Staff costs for program expansions/enhancements;
- Translation services for materials and/or public meetings;
- Printed materials and education videos;
- Professional program evaluation services;
- Mileage reimbursement and/or travel costs directly related to client outreach (mileage at the current year’s IRS reimbursement rate);
- Other tangible and direct costs related to the project, which have been justified in the budget narrative as essential for project success; and
- Administrative overhead is capped at 10% of total funding requested and includes but is not limited to the following:
- Fuel, electricity and utilities costs;
- Rent and facility costs;
- Computer/tech support;
- General office supplies;
- Taxes, insurance, and licensing fees;
- Indirect labor costs (accounting, human resources, etc.)
- In a consortium model only, lead applicants may request an additional 5% for administrative overhead for a total administrative cap of 15% of total funding requested.
Eligibility Criteria
All applicants must serve children, pregnant women, senior citizens, the chronically ill, and/or people with respiratory/ cardiopulmonary illnesses or impairments in the Priority/Eligibility Zone(s). In addition, the applicant must be a government agency; a 501(c)3 nonprofit organization that provides health education, outreach or services; or a licensed health care provider such as a hospital or clinic.
