Transformation Grants are to help strengthen the social and public sectors so the Foundation’s mission can be realized.
- Create profound system changes that benefit the residents of the South Lake County service area by establishing partnerships and collaborating with nonprofit organizations.
- Enhance the long-term Health and Wellness of the community by confronting any of the healthcare issues as identified in the Community Health Needs Assessment. (i.e. Access to primary care: medical and dental, mental illness, substance abuse, obesity, diabetes, access to food and nutrition, et al.).
- Have detailed descriptions of the projects goals and strategies including any collaborating partnerships, and a budget narrative that is sustainable over multiple years.
- Either launch new initiatives and services, expand current successful projects, or collaborate with appropriate healthcare agencies in compliance with the founding documents of the Live Well Foundation of South Lake County.

The Live Well Foundation of South Lake welcomes grant requests from otherwise eligible tax-exempt organizations under Internal Revenue Code Sections 501(c)(3) and Section 509(a) providing services within the South Lake region: Clermont, Groveland, Mascotte, Montverde, Minneola, and the Four Corners and other unincorporated areas serving South Lake County, Florida.
Eligibility Criteria
The Live Well Foundation of South Lake will financially support, through a grant process, organizations that are:
- Mission compatible
- 501(c)3 tax-exempt not-for-profit organizations or and Section 509(a) working to enhance health and wellness in South Lake County, Florida to include:
- Clermont
- Groveland
- Mascotte
- Montverde
- Minneola
- Four Corners area serving South Lake County, Florida
- Unincorporated South Lake County
- Focusing on the same areas of interest as the Live Well Foundation of South Lake to include:
- Access to health and wellness services
- Advocacy for health and wellness
- Medical services
Review and Selection Criteria
- The proposal strengthens the organization’s ability to improve population health in South Lake County.
- The organization has the necessary partnerships with consultants, experts, or partner organizations to complete the proposed project.
- The organization’s infrastructure (including governance, leadership, staffing, program, and fiscal management) is adequate to ensure the grant’s success.
- There is a sustainability plan to ensure the project continues after the Foundation’s support ends.
- The proposal aligns with the Foundation’s mission to inspire sustainable initiatives that enhance the health and wellness of the South Lake Community.
- The evaluation plan is detailed enough to assess the project’s success.
- The budget is clearly aligned with the proposed activities.
