The purpose of this funding opportunity is to:
- Prevent at-risk households from being evicted and experiencing homelessness;
- Rapidly re-house individuals and families who are currently homeless;
- Provide assistance to emergency shelters for general operations and case management services;
- Provide street outreach, emergency care, and services to unsheltered persons; and
- Administer the Homeless Management Information System Administration (Eligibility Restricted to COC-Lead Agency Only).
Funding intends to support agencies that adhere to housing-focused evidence-based and best practices, adopt Wake County’s Bridge to Home (B2H) model and the community-developed Bringing Neighbors Home Strategy to address unsheltered homelessness, and participate in the Wake County Continuum of Care’s Coordinated Entry and Homeless Management Information System (HMIS). The goal of this funding is to create and strengthen a system that locally will make homelessness rare, brief, and nonrecurring.
Grant Period
The period the funding will cover is July 1, 2025—June 30, 2026, with possible annual renewals for Fiscal Year 2026-2027 and Fiscal Year 2027-2028.
Eligibility Criteria
Only applications from nonprofits with 501(c)(3) designation from the federal Internal Revenue Service (IRS) will be reviewed.
