The Elevance Health Foundation will prioritize funding for programs that improve maternal/ infant health, target disparities, address social needs, and remove barriers to care throughout the pregnancy journey—from pre-conception support to postnatal care for mothers and babies.

Goals 

Elevance Health Foundation is requesting proposals for programs and initiatives that support one or more of the following goals: 

  • Reducing disparities in pre-term birth rates 
  • Reducing disparities in severe maternal morbidity (SMM) incidence 
  • Improving maternal/infant health outcomes

Priority Considerations 

Priority consideration will be given to grant programs that:

  • Support whole-person health
  • Address the health-related social needs of those being served
  • Include interventions that are culturally relevant to those being served 
  • Build trust through programming that considers the unique needs of those being served
  • Offer innovative, scalable solutions 
  • Generate measurable outcomes

Types of Programs

Elevance Health Foundation will consider proposals from qualified nonprofit organizations across the U.S. with an emphasis on: 

  • National programs. Promoting scalable and sustainable systemic change. or 
  • Local programs. Implementing relevant interventions in California, Florida, Georgia, Indiana, Missouri, Nevada, New York, Ohio, Texas, and Virginia

Grant Term

1–3 years based on need (12-month increments).

Eligibility Criteria

Applicants must meet the following eligibility criteria: 

  • Proof of status as a registered 501(c)3 charitable organization under one of these Internal Revenue Code subsections:
    • 170(b)(1)(A)(vi). Receives substantial support from governmental units or the general public
    • 509(a)(2). Normally receives no more than one-third of its support from gross investment income and unrelated business income, and more than one-third from contributions, fees, and gross receipts related to exempt purposes
    • 509(a)(3). Operated solely for the benefit of and in conjunction with other exempt organizations (public charities): Type I, Type II, or Type III functionally integrated 
  • Clearly define measures and accountability standards, including key milestones 
  • Track and record long-term outcomes of program effectiveness and sustainable change

Requirements 

Successful grant applications will be required to provide semi-annual reports demonstrating progress towards achieving metrics specific to the program goal. Such metrics may include: 

  • Improved pre-term birth rates among socially vulnerable populations 
  • Reduced SMM incidence (adverse outcomes) among socially vulnerable populations 
  • Improved maternal/infant health outcomes such as those demonstrated through: 
    • Increased access to preconception, prenatal, and postnatal care and services 
      • Expansion of maternal health workforce
      • Increase in care and services offered through non-traditional healthcare settings (i.e. mobile clinics) 
      • Increase in access to telehealth/digital tools and services
    • Pre-conception
      • Increase in access to family planning services/resource
      • Increase in support for physical, mental, and social needs leading up to pregnancy
    • Prenatal/Delivery
      • Increase in prenatal care visits
      • Increase in support for physical, mental, and social needs leading up to and during delivery
    • Post-natal (up to 12 months)
      • Increase in postnatal care visits
      • Increase in support for physical, mental, and social needs following delivery 
      • Increase in access to family planning resources
      • Increase in individuals breastfeeding
      • Increase in well-child visits
      • Increase in infant immunization
