Criteria
In addition to the appropriate Application form, applicants must submit the following items to be considered.
- Organizational Chart indicating organizational structure and staffing levels. Organizations which supplied this the previous year may omit this.
- Proof of non-profit or governmental status. Organizations which supplied this the previous year may omit this.
- Audited Financial Statements from the most recent available Calendar or Fiscal (Note: Financial Statements must also include signed letter from the auditor summarizing findings and recommendations of the audit.) Small organizations which do not have audited financial statements should submit such statements from their umbrella organization, if applicable.
