The District of Columbia Department of Health (DC Health) is accepting proposals from qualified applicants to deliver services in key public health areas. This funding opportunity supports Title V Program priorities that aim to improve health outcomes for mothers, children, and adolescents.
The grant focuses on reducing maternal morbidity and mortality during the postpartum period, enhancing postpartum care coordination, and ensuring timely follow-up appointments. It also supports breastfeeding programs, mental health services—including trauma-informed care—for adolescents and school health staff, and promotes the use of medical homes for Children and Youth with Special Healthcare Needs (CYSHCNs).
DC Health anticipates awarding a total of $1,750,000. Annual grant amounts will range from $50,000 to $250,000. Individual awards may fall between $100,000 and $500,000 depending on the project scope and duration.
Eligible applicants include nonprofit organizations, primary care clinics, Federally Qualified Health Centers (FQHCs), and for-profit clinics that provide primary care services. The deadline to submit proposals is July 29, 2025.
