    • Necessary home modifications for persons with disabilities and/or seniors to maintain permanent housing.
  • Mental Health Access
    • Case management/housing navigation
    • Emergency shelter
    • Transitional shelter
  • Shelter Services
    • Culturally appropriate services/interpretation
    • Subsidized cost for clients.

Eligibility Criteria

  • Agencies must have 501(c)(3) nonprofit status
  • Agencies must provide services to Allen County residents
  • Agencies must report on required metrics
  • Funds cannot be used for capital projects.
