The Access to Care Empowerment (ACE) for Sickle Cell Grants Program will be focused solely on CBOs. CBOs play an important role when it comes to SCD patient care including, but not limited to, patient empowerment, peer-to-peer education, healthcare navigation for patients and caregivers and capacity building for partner organizations.
Suggested Access to Care Empowerment programs may include, but are not limited to:
- Peer-to-peer educational programs focused on patient empowerment and knowledge sharing.
- Community outreach models using health workers to directly facilitate patient access to care.
- Educational initiatives focused on patient empowerment/advocacy in the clinic (non-treatment focused).
- Capacity building initiatives with partner organizations.
- Transition of Care: Pediatric to adult care.
Funding Information
Grant awards may be up to $50,000 during a 12-month period.
Eligibility Criteria
To be considered for support, applicants must:
- Be a U.S.-based nonprofit organization dedicated to Sickle Cell and are exempt from taxation under section 501(c)(3) of the Internal Revenue Code. Proof of tax-exempt status will be required.
- Specific to community-based organizations, and not institutions.
- Have the organizational capacity needed to manage the proposed project’s timeline and deliverables.
- Only one grant proposal application per organization will be considered.
- Previous ACCEL awardees may apply, but preference will be given to new program initiatives.
