The Health Foundation for Western & Central New York has announced a new partnership with Univera Healthcare on the health plan’s 2024 Health Equity Innovation Awards, which provide funding to community-based organizations to support health and wellness programs that address racial and ethnic health disparities in Western New York.

This is the fourth year Univera Healthcare is offering Health Equity Innovation Awards, but the first time the Health Foundation will partner to provide additional funds to enhance the impact of the program.
Priority categories for the Health Equity Innovation Awards include:
- Improved maternal health outcomes
- Behavioral health or mental health conditions
- Acute and chronic disease management
- Social determinants of health such as economic stability, education access, and quality, health care access and quality, neighborhood and built environment, and social and community context
Awards Criteria
They encourage applicants to propose new and innovative ways to improve health. Health Equity Innovation Award proposals must include:
- Clear, defined goals for reducing health disparities and improving health equity for people that face a higher burden of health inequities and social disadvantages
- Specify how funding will measurably assist in improving health equity outcomes
Application proposals should focus on reducing health disparities in racial, ethnic, LGBTQ communities, people with disabilities, rural or urban communities, or those at high risk for medical issues, behavioral/mental health conditions, or negative outcomes from the previously mentioned, including death or suicide.
Eligibility Criteria
Nonprofit 501(c)(3) organizations in Univera Healthcare’s eight-county service area are invited to apply for funding of up to $30,000. That includes community-based organizations in Erie, Niagara, Orleans, Genesee, Chautauqua, Cattaraugus, Allegany, and Wyoming counties.
- Applicants must be a 501(c)(3) nonprofit organization and located in our health plan region
- Univera Healthcare does not fund political activities and/or capital campaigns
- Recipients will be announced mid-November and will receive follow-up six months after funding is received to understand impact and use of funds
