The goal of the SMMT Pilot Program is to eliminate racial and ethnic health disparities and improve health outcomes for eligible patients.
The grant will establish SMMT Pilot Programs in Duval and Orange counties to use telehealth to provide enhanced maternity care to eligible pregnant and postpartum patients in accordance with section 383.2163, Florida Statute. Each SMMT Pilot Program will use telehealth to assess eligible patient’s service needs and gaps, link eligible patients to social services, medical care, and education, coordinate care, provide for services not covered by insurance, and provide Eligible Patients telehealth tools for wellness checks. Each SMMT Pilot Program will also provide health equity and telehealth training for participating health care practitioners and perinatal professionals. The SMMT Pilot Programs will expand the use of technology-enabled collaborative learning and capacity building models to improve maternal health conditions.
