The Tennessee Department of Health hereinafter referred to as State or Department is soliciting grant proposals for a Rural Healthcare Center of Excellence.

Details:

The purpose of this Request for Grant Proposal (RFGP) is to define the State’s minimum requirements, solicit grant proposals and gain adequate information from which the State can evaluate the services you propose to provide.

Assessing rural health care facility and provider service lines to identify “core” service expansion opportunities and/or partnerships that promote “core” service delivery, including:

  • Primary care;
  • Behavioral health care;
  • Emergency services;
  • Maternal health care;
  • Oral health services / dental care; and
  • Outpatient services (e.g., urgent care, community health centers).

Types of Grants

  • Planning Grant: Funding to support complex technical assistance needs that exceed COE staff expertise and/or capacity. The COE would distribute funding to the rural community or provider and coordinate access to additional subject matter experts (i.e., third-party entity).
  • Implementation Grant: Funding to implement the service line changes, alternative payment models, or service delivery models identified through technical assistance.

Funding Information

  • Maximum grant amount of $2,400,000.
  • Planning Grant: Total Annual Budget: $400,000 per year for five years.
  • Implementation Grant: Total Annual Budget: $1,000,000 per year for five years.

Grant Period

The State intends to award up to One (1) grant(s) for a period of Twenty-four (24) months with an expected effective period from July 15, 2025 to December 31, 2027.

Allowable Expenses

  • Projects that help Tennessee providers deliver integrated, high-quality, comprehensive prevention and healthcare services, including wrap-around services, chronic disease management and behavioral health
  • Investments in evidence-informed, innovative care delivery models that improve access to health and healthcare services for rural populations
  • Projects that support transition to alternative care models to address community need
  • Projects that consider service integration programs or social service linkages to address community drivers of health
  • Projects that expand access to health and healthcare services rural populations
  • Contracts for services in support of the approved project
  • Assistance may include direct strategic, operational, and administrative support, such as:
    • Facilitating service line assessments for rural health care facilities and providers (e.g., behavioral health, urgent care, OB/GYN and/or dental “hub and spoke”)
    • Payment model assessments,
    • Delivery model / payment model transition supporting enhanced service availability and viability, and
    • Operational and financial advising.

Eligibility Criteria

Eligible Respondents to apply for this funding include the following:

  • Academic Medical Centers;
  • Health Care Non-Profits;
  • Health Care For-Profits;
  • Primary Care Clinics;
  • Safety-Net Providers; and
  • Other Health and Health Care Service Providers with demonstrated experience and success in rural health care delivery.
