The Elevance Health Foundation is prioritizing funding for programs that improve clinical health outcomes for individuals with diet-related conditions. It also aims to increase access to nutritious foods for populations experiencing food and nutrition insecurity.
This grant opportunity is open to organizations across all states and counties in the U.S. The foundation supports both national programs that promote scalable and sustainable systemic change, as well as local programs implementing relevant interventions in specific states including California, Florida, Georgia, Indiana, Missouri, Nevada, New York, Ohio, Texas, and Virginia.
Priority will be given to proposals that support whole-person health and address the health-related social needs of the people served. Programs that include culturally relevant interventions and build trust by considering unique community needs are especially encouraged. The foundation looks for innovative, scalable solutions that generate measurable outcomes.
Grants can be awarded for a period of one to three years, depending on need, with funding provided in 12-month increments.
Eligible applicants must be registered 501(c)(3) charitable organizations under specific IRS categories. These include organizations that receive substantial support from government or the public, those that receive limited income from unrelated businesses, or those operated in conjunction with other exempt public charities.
Applicants are required to clearly define measures and accountability standards, including key milestones. They must also track and record long-term outcomes to demonstrate program effectiveness and sustainable change.
