The Senior Wellness Pilot Program Grant is designed to build capacity for community initiatives that will improve socialization and support the health and wellbeing of older adults, individuals with disabilities who are homebound, and residents of institutional settings in New Jersey.
Social participation and inclusion are vital components of an age-friendly New Jersey and can mutually benefit the health and wellbeing of both socially isolated older adults and the people who interact with them. The program is designed to advance efforts that enable sustained social connections among this at-risk population, resulting in measurable improvement in wellbeing.
Funding Information
- A total of four million dollars ($4,000,000) is available through this RFP. All grants are contingent upon funding availability.
- Grants may be awarded up to $250,000 per grantee for a total of $4,000,000.
Eligibility Criteria
To be eligible for consideration for this RFP, the bidder must satisfy the following requirements:
- The bidder must be a nonprofit or governmental entity;
- The bidder may be a current recipient of DHS grant funding, including the Age Friendly Community Grants, but must demonstrate that their proposed grant activities do not overlap in any way with their existing DHS-funded projects;
- Funds may be used to support collaborations or consortia between multiple organizations. Each proposal should identify the primary bidder and any partners that will be funded by this grant, if awarded;
- The bidder must be in good standing with DHS if it has an existing grant or contract in place. If applicable, the bidder must have all outstanding Plans of Correction (PoC) for deficiencies submitted to DHS for approval prior to submission;
- The bidder must be fiscally viable based upon an assessment of the bidder’s audited financial statements. If a bidder is determined, in DHS’ sole discretion, to be insolvent or to present insolvency within the twelve (12) months after bid submission, DHS will deem the proposal ineligible for grant award;
- Pursuant to DHS Contract Policy and Information Manual Policy Circular 8.05, the bidder shall not have a conflict, or the appearance of a conflict, between the private interests and the official responsibilities of a person in a position of trust.
